Please print or fill out the PDF provided at the link below. There are 3 ways that you can send us your registration form:
1. Attach the PDF to an email by clicking this link
2. Fax us your form by using this FAX number - (855)325-0055 - ATTN: Ed Smith
3. Send your form via U.S. mail to CAICH | KUEA Bldg. - 2nd Floor | 3901 Rainbow Blvd. | Kansas City, KS 66160
Note: We have 2 annual pow wows. The O.N.E. Pow Wow in May and the Our Futures are Sacred Pow Wow in November. Please specify which pow wow for which you are interested in vending.
Please remember that no pets are allowed in the building and no food is allowed in the gym at any time. We have always mandated that all vendors comply with the Arts and Crafts act. This is enforced. Failure to comply with this will result in removing your booth and forfeiting your vendor fee. In the meantime, if you have any needs, questions or concerns, please do not hesitate to let me know.
